Getting Started

Learn about our services and how to get started.

We provide person-centred NDIS supports, including:

  • Daily Living Support
  • Community Participation
  • Mental Health and Psychosocial Support
  • Respite and Short Term Accommodation (STA)
  • NDIS Nursing Care
  • High Intensity Support
  • Accommodation options including Supported Independent Living (SIL), Specialist Disability Accommodation (SDA) and Medium Term Accommodation (MTA)

Every support is tailored to the participant’s goals, needs and NDIS plan.

Most of our supports are funded through the NDIS. If you don’t have an NDIS plan yet, or you’re unsure what your plan covers, contact our team. We can talk through your situation and point you in the right direction. [If you offer privately funded support, add: “We can also discuss privately funded options.”]

We support participants across Frankston, Carrum Downs, Seaford, Cranbourne, the Mornington Peninsula and South East Melbourne. We also have accommodation options in selected other Melbourne locations. If you’re unsure whether we cover your area, give us a call on 1300 659 621.

Accommodation

Questions about SIL, SDA, MTA and housing options.

Yes. Participants, families, nominees and Support Coordinators are welcome to arrange a private inspection. Let us know beforehand about any mobility, communication or sensory needs so we can make the visit comfortable. For residents’ privacy, we share exact addresses once an inspection is confirmed.

We offer:

  • Supported Independent Living (SIL): support with daily living in a shared home
  • Specialist Disability Accommodation (SDA): homes designed for people with extreme functional impairment or very high support needs
  • Medium Term Accommodation (MTA): while you wait for a long-term home
  • Short Term Accommodation (STA) and respite

We also help with hospital-to-home transitions.

Referrals

How to make a referral and what to expect

Complete our online referral form, call us on 1300 659 621, or email our team. Participants, families, carers, Support Coordinators and health professionals can all make a referral. Please make sure you have the participant’s consent before referring.

We aim to respond to all referrals within [1 business day]. For urgent referrals, such as a hospital discharge or a housing breakdown, please call us directly on 1300 659 621.

Helpful information includes:

  • the participant’s name and contact details
  • your details as the referrer
  • the supports required
  • how the NDIS plan is managed
  • a brief reason for the referral
  • confirmation of the participant’s consent

If you have them, a copy of the NDIS plan, relevant reports, Behaviour Support Plans or risk information helps us respond faster. Don’t worry if you don’t have everything; we can gather the rest together.
